About the position

The Community Health Initiatives Manager (CHIM) plays a crucial role in the Illinois Primary Health Care Association (IPHCA) by developing, implementing, and monitoring various grant-funded programs aimed at enhancing community health services. This position supports the Vice President of Clinical Services & Workforce Development and other senior leadership staff in various projects, ensuring that the initiatives align with the organization's goals and the needs of its member health centers. The CHIM is also tasked with identifying and pursuing additional funding opportunities to expand programs and services that benefit IPHCA member health centers. In addition to program management, the CHIM represents IPHCA at various stakeholder committee meetings, work groups, and task forces, contributing to discussions and decisions that impact community health initiatives. The role requires strong interpersonal skills, as the CHIM must effectively communicate with a diverse membership and prepare various forms of communication, including grant reports, newsletters, and external communications. The ability to plan and manage meetings, prepare agendas, and facilitate discussions is essential for success in this position. The CHIM will also be responsible for developing training materials, conducting surveys, and preparing reports that analyze community health needs and program effectiveness. This position requires a commitment to fostering member relations and ensuring quality assurance in customer service, reflecting the values of the IPHCA.

Responsibilities

  • Develop, implement, and monitor grant-funded programs related to community health initiatives.
  • Support the Vice President of Clinical Services & Workforce Development with various projects.
  • Seek out additional funding opportunities for programs and services that benefit IPHCA member health centers.
  • Represent IPHCA at stakeholder committee meetings and work groups.
  • Prepare grant reports and external communications for members and non-members.
  • Plan and manage meetings, including preparing agendas and meeting reports.
  • Lead and facilitate discussions on community health topics.
  • Prepare training materials and course outlines for community health initiatives.
  • Conduct surveys and prepare reports analyzing community health needs and program effectiveness.
  • Maintain awareness of policy priorities and prepare issue briefs.

Requirements

  • Graduate degree in public health or social services.
  • Two or more years of experience in developing and implementing community health or grant-funded projects for federally qualified health centers or safety net clinics.
  • Strong interpersonal and communication skills, both verbal and written.
  • Familiarity with state/local/foundation public health grant processes.
  • Knowledge of quality improvement concepts and health information technology.
