About the position

The Community Health Manager plays a crucial role in overseeing the Community Health Department under the direction of the Tribal Administrator. This position is responsible for managing health care delivery systems, program planning, daily operations, and the implementation of departmental procedures. The Community Health Manager will supervise staff, manage budgets, and evaluate departmental programs while ensuring compliance with grant requirements and reporting. Additionally, the role involves identifying and securing future funding opportunities and ensuring that policies and procedures are current and reflect best practices in community health management. In this position, the Community Health Manager will plan, develop, and direct the implementation of goals, objectives, policies, and work standards for community health programs. The manager will oversee daily activities, monitor community health care needs, and evaluate the effectiveness of programs and services. Building and maintaining positive relationships with community and tribal members is essential, which includes conducting outreach, attending tribal events, and ensuring that staff carry out their duties in a professional and culturally appropriate manner. The role also requires managing program objectives and grant requirements, including timely reporting and compliance with funder requirements. The Community Health Manager will assist in preparing the operating budget, monitor expenditures, and approve requests in line with financial processes. Educating the community about the department's services and events is also a key responsibility. The manager will utilize external resources to assess existing programs and improve service efficiency and quality. Personnel functions, including hiring, training, and evaluating staff, are also part of the role, along with attending conferences and trainings to stay updated on policies and industry trends.

Responsibilities

  • Plans, develops, organizes, and directs the implementation of goals, objectives, policies and work standards for community health programs.
  • Oversees the daily activities of the community health programs, monitors changing community health care needs and evaluates the ability to meet those needs through monitoring and ongoing evaluation of programs, services and departmental staff.
  • Maintains positive relationships with community/tribal members by conducting outreach, attending tribal events, and ensuring that home visits, transports, community groups, events and other case worker duties are being carried out by staff in a professional and culturally appropriate manner.
  • Manages and ensures successful completion of program objectives and grant requirements, including reports, work plans, and data collection.
  • Ensure compliance for all existing Community Health Services Grants, seek new grants to enhance services and follow HBPI grant seeking process to secure new grants and/or funding.
  • Responsible for the accurate and timely reporting on activities in accordance with funder requirements.
  • Assists in preparation of operating budget for the department, monitors the budget for expenditure compliance and approves authorized expenditure requests in accordance with HBPI financial processes.
  • Educates the community on the Community Health Department's services, events and activities.
  • May utilize external resources and analysis tools to assess the structure of the Department's existing programs to improve efficiency and quality of services while assuring operational effectiveness.
  • Oversees all personnel functions of departmental staff, including hiring, training, evaluating and discipline.
  • As appropriate, incorporates staff development plans that include education, training, conferences, workshops, etc.
  • Fills in for staff as needed.
  • Recognizes critical situations and takes appropriate action. Follows up on problems, decisions and requests.
  • Delegates assignments effectively and follows up to ensure completion.
  • Networks and maintains ongoing relationships with other departments, other tribes, health care and social services providers of CTHP and County service providers.
  • Attend conferences and trainings to stay current with policies and industry trends (Annual IHS conference).
  • Attends required meetings and trainings and facilitates departmental meetings.
  • Ensures the dissemination of pertinent information to the Tribal Administrator.
  • Adheres to a strict confidentiality standard.
  • Other duties as assigned.

Requirements

  • An associates degree in public health administration, Social Services, Sociology, Psychology or related field and two (2) years working as a manager or assistant manager of a similar sized public health, mental health or social services department or five (5) years of successful and progressively responsible full-time management and administrative experience in a public health, mental health or social services agency which included responsibility for fund, grant and budget administration, grant writing/grant management, program planning, implementation and administration.
  • A Bachelor's Degree in Public Health Administration; Social Work, or related field is highly desirable.
  • Valid California Class C Driver's License and be insurable for HBPI auto insurance, current CPR, First Aid and Mandated Reporter Certification.
  • Familiarity with community development principals and tools that strengthen community participation; ability to manage multiple tasks and meet deadlines; maintain confidentiality at all times; actively involved in the community and understanding of the Native American Culture.
  • Proficient in Microsoft Office Suite: Word, Excel, Power Point, and Outlook.
  • Excellent writing, oral and interpersonal communication skills and an intuitive listener; including conflict resolution and crisis intervention; ability to interact with clients from varied backgrounds, age groups and with a variety of health care needs; ability to prepare clear and concise reports, correspondence, grants, policies and other written materials; exercises sound independent judgment within policy guidelines.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to apply common sense understanding during normal and stressful conditions.
