About The Position

The Community Health Outreach Coordinator at Wheeler Clinic is responsible for engaging the community in health services and activities, providing education, outreach, and support to individuals and families. This role aims to enhance access to primary and behavioral health care, reduce disparities in care, and promote health equity through various community engagement initiatives.

Requirements

  • High School diploma or GED required.
  • 3 years of experience in community organizing, marketing, prevention/health promotion, or other social service fields.
  • Valid Driver's License required.
  • Ability to work effectively in a team and communicate effectively both verbally and in writing.
  • Strong organizational skills and ability to manage time effectively.

Nice To Haves

  • Experience in health promotion or community health outreach is preferred.

Responsibilities

  • Participates in the planning, coordination, and implementation of community engagement, education, outreach, and support activities.
  • Conducts outreach and education activities including prevention, health promotion, and wellness training events.
  • Coordinates and facilitates community training events and programs for all health center locations.
  • Strengthens existing collaborations and establishes new partnerships and patient referral sources in the community.
  • Plans and participates in community events and initiatives to enhance health outcomes.
  • Represents Wheeler Health & Wellness in the community as needed.
  • Coordinates access to resources and advocates for individuals and families as needed.
  • Maintains records of coordination of care and completes documentation for client and program records.
  • Supports daily program-specific operational functions as required.
  • May transport clients to appointments or linkages as appropriate.
