About the position

The Community Health Partner (CHP) role involves working closely with RN Care Managers to provide integrated social care for members with complex health needs. This position focuses on care coordination, helping members navigate community-based services, and ensuring effective communication between medical care providers. The role is primarily based in Montgomery, Green, or Clark County and requires significant interaction with members through various communication methods, including telephonic and in-person visits.

Responsibilities

  • Engage with members from care teams and describe program expectations and goals.
  • Complete assessments and screening instruments, including for behavioral health disorders.
  • Collaborate with RN Care Manager to determine member placement in appropriate programs.
  • Develop care plans in partnership with RN Care Manager and incorporate quality opportunities.
  • Support members in achieving their care plan goals and address social needs.
  • Participate in case conferences and follow up with members on care coordination needs.
  • Provide routine non-clinical education on preventative care topics to members.
  • Utilize care facilitation and electronic health record systems for documentation and communication.

Requirements

  • High School Diploma required.
  • Unrestricted Driver's License and vehicle for daily use.
  • 1-2 years of experience in community care or care coordination required.
  • Comfortable using technology for member support without in-person contact.
  • Understanding of scheduling platforms for accurate appointment management.
  • Proficient in collecting and documenting member clinical and demographic data.
  • Versed in Motivational Interviewing and Trauma Informed Care principles.
  • Strong problem-solving skills and ability to collaborate with team members.
  • Adaptable to change and prepared for fast-paced work environments.

Nice-to-haves

  • Experience with electronic health record systems and care facilitation platforms.
  • Knowledge of community resources and social services available to members.
