About the position

The Community Health Program Coordinator will play a vital role in the Comer Mobile Unit Program, which provides essential primary care services to children, schools, and communities, primarily on the Southside of Chicago. This position involves assisting the Principal Investigator (PI) in the development of a Mobile Unit Program dashboard, which includes tracking clinical activities, performing data abstraction, and conducting data analysis. The coordinator will also be responsible for establishing and maintaining important relationships with community health centers, sub-specialty providers, and various community-based resources to enhance patient access to healthcare services. In addition to these responsibilities, the Community Health Program Coordinator will work closely with the Comer Mobile Unit team to streamline resources and ensure the effective delivery of healthcare and social services. This role requires a proactive approach to familiarizing staff and patients with available resources, such as a comprehensive web-based database for health and social services on Chicago's South Side. The coordinator will also be tasked with abstracting subject data from medical records and entering it into Redcap databases, tracking referrals, and conducting follow-ups to ensure patients receive necessary services. The position is funded by contractual grant funding, which is subject to renewal based on the grantor's provisions. Therefore, employment is contingent upon the continued receipt of these funds and satisfactory job performance. The coordinator will also manage patient incentive programs, maintain equipment and supplies, and assist in monitoring compliance with regulatory policies. Overall, this role is crucial for enhancing the health outcomes of children and families in the community by ensuring they have access to the necessary healthcare services and resources.

Responsibilities

  • Develop relationships with community health centers, sub-specialty providers, and community-based resources to facilitate patient access.
  • Familiarize staff and patients with resources such as southsidehealth.org, enabling access to food pantries, housing support, and mental health services.
  • Abstract subject data from medical records and enter it into Redcap databases for tracking referrals and assessments.
  • Assist in creating a Mobile Unit Program dashboard by tracking clinical activity.
  • Conduct literature reviews as needed.
  • Track all referrals and follow up to ensure patients receive necessary services.
  • Follow up with families to address barriers to care, such as transportation issues.
  • Coordinate with sub-specialty offices to obtain consultation letters for patient records.
  • Facilitate enrollment in All Kids, Illinois's medical coverage program.
  • Manage the patient incentive program, including distributing incentives to families.
  • Maintain and order equipment and supplies for the Mobile Unit.
  • Monitor the overall progress of the project in collaboration with the PI and team members.
  • Plan and implement systems to optimize project results with the PI.
  • Monitor compliance with institutional, state, and federal regulations.
  • Track vaccine inventory and maintain VFC vaccine supply, including monitoring storage temperatures.
  • Prepare and submit vaccine inventory reports.
  • Scan and upload medical documents into EPIC, ensuring completeness of visit information.
  • Provide technical and administrative support for research projects, including data collection and analysis.

Requirements

  • Minimum vocational training or equivalent experience in a related field.
  • 2-5 years of work experience in a related job discipline.
