The Community Health Program Manager is an integral position to the operations of the Community Health Department within SNAHC. It is responsible for overall management of programs and activities targeted to families and adults, and will also be responsible for resource management, including the management of grants that fund services. The Program Manager has management and responsibility over grant objectives and deliverables, which include programming, evaluation, and budget/financial management. The Program Manager will continue to strengthen community participation in program design, evaluation, and policy advocacy. The program manager will work closely with other department managers to ensure programs and activities are aligned with broader department and organizational goals focused on health equity.

Responsibilities

  • Manage the implementation and evaluation of federal and non-federal grant programs focused on the American Indian and Alaska Native community and target populations.
  • Oversee and complete grant projects and deliverables, as well as reporting requirements.
  • Supervise and develop staff, including grant project coordinators who implement grant deliverables.
  • Collaborate with project evaluators regarding epidemiological, evaluation, and outcome factors.
  • Develop assessment and evaluation tools in target communities to benefit grant activities.
  • Ensure documents and promotional material comply with branding policy and are suitable for internal/external communication.
  • Establish, expand, facilitate, and sustain initiatives, programs, and partnerships that engage key stakeholders and community members.
  • Support SNAHC leadership in cross-department processes and systems, including the development of organizational initiatives and policies.
  • Train staff and stakeholders in a culturally and linguistically sensitive manner.
  • Act as an advocate and liaison between community members and the larger system of care.
  • Represent SNAHC at the federal, state, and county levels.
  • Ensure direct reports adhere to department and agency policies, procedures, and protocols, including HIPAA compliance.
  • Perform management duties including recruiting, onboarding, timekeeping, performance evaluations, coaching, and progressive discipline as needed.
  • Participate in and guide internal quality improvement teams.
  • Drive quality improvement initiatives in accordance with the mission and strategic goals of the organization.

Requirements

  • 3 years of experience in managing federal and non-federal health grants, including grant implementation, evaluation, and fiscal management.
  • 2 years of experience supervising multiple staff members.
  • Desire to work closely with families and youth.
  • Familiarity with historical trauma and its effects on individuals, families, and communities.
  • Respect for and knowledge of traditional, cultural, and spiritual practices of a diverse Native American community.
  • Current California driver license, a dependable vehicle, and valid vehicle insurance as required by CA state law.
  • Community organizing experience.

Nice-to-haves

  • Experience in an FQHC setting as a member of a multi-disciplinary team preferred.
  • Knowledge of traditional, cultural, and spiritual practices of the diverse AI/AN community.
  • Bilingual.

  • Competitive salary range of $58,573 - $79,246 based on experience and qualifications.
