About the position

The Community Health Program Manager at Heart to Heart International (HHI) is responsible for supporting and executing the goals of the organization's local Community Health initiatives. This role focuses on improving healthcare access for vulnerable populations through collaboration, advocacy, and customized solutions, deploying community health workers and licensed professionals to provide essential services. The position requires effective management of program activities, direct reports, and community engagement to enhance health outcomes in underserved areas.

Responsibilities

  • Identify and work alongside resource development on grant proposals to secure funding for programs, helping ensure sustainability and growth.
  • Assist in tracking budget allocation and program spending to uphold fiscal responsibility.
  • Manage community health program activities to augment partner organizations' capacity and reach in providing healthcare services to uninsured and underinsured individuals targeting improved health outcomes in chronic and acute disease management.
  • Manage direct reports, including Community Health Workers and Licensed Registered Nurses, providing leadership, guidance, and support.
  • Manage vaccine inventory and adhere to cold-chain protocols as per state and CDC requirements.
  • Ensure clinical documentation is organized, and HIPAA regulations are followed.
  • Engage in community events.
  • Work and communicate effectively and respectfully with highly diverse populations.
  • Promote utilization of HHI cross-departmental resources in underserved areas.
  • Attend ongoing training to stay current with the latest information on continued education for community workers.
  • Support the creation of training materials for Community Health Workers (CHWs) and Licensed Practitioners (LPs).
  • Promote Community Health Program through presentations and discussion panels to increase awareness and engagement.
  • Assess and share identified equity gaps with the Community Health Programs Director for programmatic considerations.
  • Other duties as assigned.

Requirements

  • A bachelor's degree in Community Health, Public Health, Clinical Licensed Practitioner (RN, NP, Pharmacist) related/relevant fields and/or 3 years' experience working in a relevant field (will accept a combination of education and experience)
  • Successful completion of a drug screen and background check
  • Proof of COVID-19 vaccination(s), Hep B and Flu
  • Valid Driver's License (clean driving record)
  • Ability to drive a large vehicle
  • Flexible schedule, available for possible evening and weekend hours
  • Available for travel
  • Lift and carry 40 lbs.
  • Highly dependable, reliable, and flexible
  • Must work well in a team setting.
