The Home Health RN Case Manager at Interim HealthCare is responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services within the community. This role offers flexibility in scheduling, allowing nurses to balance their professional and personal lives effectively. The position is available in various formats including Full Time, Part Time, and Per Diem, with a focus on delivering exceptional patient care.
Responsibilities
- Manage a defined caseload of patients.
- Provide direct patient care and comprehensive nursing services.
- Coordinate with healthcare professionals to ensure quality care.
- Document patient progress and maintain accurate records.
- Educate patients and their families about health management and care plans.
Requirements
- Valid Nursing License in the State of New Hampshire.
- Nursing experience required.
- Current CPR Certification required.
