About the position

The Community Health Specialist I plays a crucial role in the Tuberculosis team, providing direct individual and community health services under the supervision of the Tuberculosis Coordinator. This position involves administrative support, health education, and direct client interaction, focusing on TB prevention and treatment in both community and clinic settings.

Responsibilities

  • Assist with and provide services to clients in community and clinic settings, including home visits.
  • Provide administrative support by scheduling appointments, verifying insurance, processing billing, and handling receipts.
  • Assist in the completion of a patient's 'Fee for Service' application.
  • Conduct interviews and assist in TB infection case investigations and surveillance.
  • Provide health education presentations related to TB contact investigations and surveillance.
  • Develop and maintain positive contacts with allied community service providers.
  • Provide follow-up and referral services as appropriate.
  • Maintain documentation of services in accordance with program procedures.
  • Submit data to designated senior staff for accountability.
  • Support CHS II staff by performing interviews and data entry into relevant systems.
  • Provide basic instruction to clients on TB disease prevention or treatment.
  • Participate in staff meetings and assigned training activities.
  • Assist in the orientation and training of new staff.
  • Participate in the development of annual program goals and assess progress towards achieving those goals.

Requirements

  • Associate degree or bachelor's degree in a related field (nursing, public health, health education, social work, psychology, sociology, communications, etc.) or 3-5 years of Communicable Disease investigation and surveillance experience.
  • Fluency in reading and speaking conversational Spanish is required.
  • Valid Illinois driver's license and automobile insurance.
