About the position

The Community Health Worker (CHW) plays a vital role in enhancing the health and quality of life for patients aged 6 and older within the communities served by Georgia Highlands Medical Services. This position focuses on improving access to healthcare by conducting outreach, providing education, and facilitating connections between patients and necessary health services. The CHW engages with community members to identify healthcare needs, offers referrals, and ensures follow-up to promote better health outcomes.

Responsibilities

  • Identify individuals and families in need of healthcare through community outreach and provide referrals to GHMS or appropriate providers.
  • Provide intensive follow-up services to current GHMS patients to ensure access to necessary medical visits and services.
  • Assist community members in navigating service systems and connecting them to the right resources, eliminating barriers to care.
  • Raise awareness about affordable insurance options and assist patients with Medicaid applications.
  • Serve as a liaison between patients, the community, and GHMS services, educating them about available programs.
  • Perform lay health care education to engage individuals in lifestyle changes and self-management.
  • Coordinate and implement community-building events and programs such as health fairs and support groups.
  • Provide home visits and community support services to existing GHMS patients in need of assistance.
  • Collect and report accurate data on patient contacts, health screenings, and referrals, documenting barriers to care.
  • Collaborate with staff to ensure an excellent patient experience and improved health outcomes.

Requirements

  • High School Diploma or GED
  • Valid Driver's License
  • 1-3 years of customer service and/or public service experience

Nice-to-haves

  • Experience in mental health counseling (1 year preferred)
  • Experience in clinical counseling (1 year preferred)
  • Bilingual in Spanish (required)
