About the position

The Community Health Worker (CHW) at Jordan Valley Community Health Center plays a vital role in promoting health equity within the community. This position involves building relationships with patients and community members to facilitate access to healthcare services and resources, ultimately improving the quality of life for individuals in underserved areas. CHWs work closely with local school districts and healthcare providers to identify needs, set health and wellness goals, and coordinate care for students and families.

Responsibilities

  • Work directly with the partnering school district to identify needs and set student/patient-centered health and wellness goals.
  • Build relationships and interact professionally with partnering school district staff, community partnerships, healthcare providers, and social service providers.
  • Identify individuals with needs in both school-based and community settings.
  • Coordinate and attend school-based events and child-centered clinic events planned by Jordan Valley and the partnering school district.
  • Schedule and execute school-based appointments such as telehealth appointments, dental exams, well child exams, and vision exams.
  • Provide referrals and follow-up on referrals after medical, dental, behavioral, therapy, or vision appointments.
  • Work directly with a patient's treatment team and participate in care coordination and/or case management.
  • Follow up on appointments or instructions from the healthcare team through school visits, home visits, community meetings, or phone communication.
  • Connect patients with resources for basic needs.
  • Assist patients and community members in navigating health and social service systems.
  • Empower and educate community members to build individual and community capacity.
  • Provide basic health screenings, such as Well Child Checks.

Requirements

  • Valid Driver's License
  • Bachelor's Degree (preferred)
  • 1-year clinical experience or in the Community Health field (preferred)
  • Trained in and/or experience in Community Health Worker field
  • Knowledge of healthcare organizations and resources
  • Understanding of lifestyle choices and child development
  • Experience in project/event organization
  • Awareness of cultural beliefs and healthcare
  • Knowledge of legal and ethical considerations
  • Familiarity with disease-specific modules (e.g., asthma/COPD, cardiovascular diseases, developmental disabilities, diabetes, overweight/obesity, tobacco use)
  • Ability to conduct interviews
