About the position

The Community Health Worker/Family Support Specialist at Nurses for Newborns is a full-time role focused on providing essential services to families in Lincoln and Warren Counties. This position involves home visits and virtual support, emphasizing cultural mediation, health education, case management, and community outreach. The ideal candidate will have a strong understanding of community resources and a commitment to improving the health and well-being of families in the community.

Responsibilities

• Visit clients in their homes or virtually as appropriate according to NFN guidelines.
• Assist with cultural mediation among individuals, communities, health, and social service systems.
• Provide culturally appropriate health education and information.
• Offer case management and system navigation support.
• Provide coaching and social support to clients.
• Advocate for individuals and communities.
• Assist in building individual and community capacity.
• Conduct community outreach activities.
• Complete documentation in the clinical database for client contacts and scheduled appointments based on NFN policies.
• Attend required Home Visitor, Program, and Case Management meetings.
• Perform other activities and duties as assigned.

Requirements

• Minimum of a high school diploma or GED.
• Demonstrated knowledge of and experience with community resources and issues.
• Community Health Worker credentialing through the State of Missouri is required or should be in progress.
• Excellent customer service skills, both internal and external.
• Effective verbal and written communication skills.
• Excellent computer skills, including data entry skills.
• Ability to establish a trusting and therapeutic relationship with clients, caregivers, and families.
