Wheeler Clinic is seeking a dedicated Community Health Worker to join our Mobile Medical Team. This position is integral to our mission of providing equitable access to innovative care that improves health, recovery, and growth at all stages of life. The Community Health Worker will play a vital role in building community partnerships and scheduling events for Wheeler's Mobile Medical Unit(s). This role involves providing education, outreach, and support to the community through our Mobile Medical Unit(s). The Community Health Worker will work collaboratively within a multi-disciplinary team to reduce disparities in care, support individuals and families with specific disease management, and increase access to primary care, behavioral health, and community resources. The Community Health Worker will be responsible for planning, coordinating, and implementing community engagement, education, outreach, and support activities, as well as primary care and behavioral health services through Wheeler's Mobile Medical Unit(s). This includes conducting health promotion and wellness training events, health screening events, recovery support services, and referrals to other community resources. The position requires the ability to safely drive and operate the Mobile Medical Unit, ensuring all necessary supplies and equipment are organized and loaded for events. The Community Health Worker will also create new patient/client charts, provide peer support, and assist individuals and families in understanding and accessing needed resources in their communities. This part-time position, requiring 20 hours per week, offers some flexibility outside of scheduled activities/events, with potential weekend hours as needed. The office location will be in Bristol, CT, but travel to various Wheeler health center locations and scheduled events is required. The ideal candidate will have a high school diploma or GED, along with at least two years of experience working with individuals and families in the community. A valid Connecticut Driver's license is essential, as is access to reliable transportation. The Community Health Worker must demonstrate effective communication skills, both verbally and in writing, and be committed to personal and program growth.
