About the position

The Community Health Worker (CHW) position at the University of Arkansas for Medical Sciences (UAMS) involves managing relationships with patients and community organizations to provide education and resources aimed at improving health outcomes. The role is structured into three levels, with increasing responsibilities from outreach and recruitment to health education and care coordination. CHW III will also mentor lower-level CHWs and participate in multidisciplinary teams to address public health challenges.

Responsibilities

  • Conduct outreach to individuals, communities, service providers, and groups.
  • Identify and recruit individuals, families, and community groups to services and systems.
  • Educate health and social service systems and providers about community perspectives and cultural norms.
  • Facilitate cross-cultural communication among individuals, communities, and health/social service system workers.
  • Coordinate care, provide case management support, and assist individuals and communities in navigating health and social service systems.
  • Participate in the design, implementation, and interpretation of community-level assessments.
  • Present at local agencies and community events to share information and educate individuals and communities about health and social service concerns and resources.
  • Motivate and encourage people to obtain care and other services.
  • Support self-management of disease prevention and management of health conditions, including chronic disease.
  • Provide culturally appropriate health education and information.

Requirements

  • High School diploma/GED for all levels.
  • Two years of general experience in community outreach, education, case management, and administration for Level I.
  • Completion of a certified CHW training program for Level II.
  • Four years of experience as a CHW for Level II.
  • CHW certification from ARCHWA for Level III.
  • Nine years of experience as a CHW for Level III.

Nice-to-haves

  • Experience in public health outreach and education.
