About the position

The Community Health Worker (Peer Support Specialist) at Home At Last CDC plays a crucial role in supporting individuals experiencing homelessness, particularly those who are justice-involved. This position focuses on providing comprehensive assessments, planning, and ongoing support to enhance client management and satisfaction. The goal is to promote continuity of care and effective case management through a trauma-informed approach, ultimately helping clients reintegrate into the community and sustain independent housing.

Responsibilities

  • Mentor, motivate, and empower clients to become positive members of their communities.
  • Assess clients at the point of enrollment and provide appropriate linkages and referrals to needed services through an individualized care plan.
  • Meet with participants in person or virtually on a weekly or bi-weekly basis and document interactions in CHAMP.
  • Identify community resources appropriate to meet participants' needs.
  • Assist participants in navigating health systems through appointment accompaniment, transportation assistance, and coaching.
  • Attend all required trainings and events organized by the county to foster learning and community.
  • Maintain a detailed case file for every client assigned for service.
  • Ensure facility cleanliness by assigning and monitoring client work, morning duty, and extra duty assignments.
  • Perform intake and attend all staff meetings and training as directed.
  • Communicate with Parole Officers or other Social Service Providers regarding the treatment, advancement, or clinical needs of the client.
  • Investigate and report adverse occurrences, and work with other staff to identify resources.

Requirements

  • Must be 18 years of age or older.
  • High School Diploma or equivalent.
  • Must complete live scan/background clearance.
  • TB Clearance required.
  • A history of incarceration with demonstrated stability and successful reintegration into the community following incarceration.
  • Prior experience working with individuals with system involvement, chronic medical issues, mental illness, SUD, or experiencing homelessness.
  • Ability to work a flexible schedule (weekends and evenings).
  • Good oral and written communication skills.
  • Ability to work effectively under stressful situations.
  • Ability to exercise initiative and sound judgment and to react with discretion under varying conditions.
  • Demonstrate a high degree of integrity and ethical standards and judgment.
  • Reliable transportation is required.
