About the position
The Community Health Worker (CHW) position at Providence is designed to support Health Homes Program (HHP) members in improving their overall health through outreach and engagement activities. This role involves collaboration with the Community-Based Care Management Entity (CB-CME) team, healthcare professionals, and patients' families to provide comprehensive care. The CHW may also support the COVID Recovery Clinic, working closely with various healthcare teams to ensure effective patient care.
Responsibilities
- Work collaboratively with the CB-CME team to support HHP members.
- Conduct outreach and engagement activities primarily in the field.
- Collaborate with primary care physicians, specialists, and therapists in the COVID Recovery Clinic.
- Assist in improving the whole health of members through effective communication and support.
Requirements
- Certification as a Community Health Worker (CHW) or 2 years of experience as a CHW, Promotora, or Health Navigator.
- 2 years of experience in community outreach, customer service, or within a medical office.
- Experience in a Behavioral Health or Substance Use Disorder program is a plus.
