About the position

The Community Health Worker I (CHW I) serves as a vital link between health/social services and the community, facilitating access to services and enhancing the quality of service delivery. This role focuses on building individual and community capacity through outreach, education, and advocacy, while promoting health knowledge and self-sufficiency.

Responsibilities

• Serve as a liaison between health/social services and the community.
• Facilitate access to services and improve service delivery quality.
• Conduct outreach and community education to increase health knowledge.
• Provide informal counseling and social support to individuals.
• Advocate for community health needs and resources.
• Build and maintain a panel of patients in collaboration with supporting departments.
• Provide culturally appropriate health education and information.
• Coordinate care, manage cases, and navigate health systems for individuals.
• Document and track individual and population-level data.
• Assist in creating a positive and supportive work environment.

Requirements

• High school diploma or equivalent.
• 1 year of healthcare experience.
• Community Health Worker (CHW) certification in Kentucky.
