About the position

The Community Health Worker at Orchid Health plays a vital role in establishing trusting relationships with patients and their families, providing support and encouragement, and engaging them in their care plans. This position focuses on addressing Social Determinants of Health (SDOH) and involves outreach, follow-up, and coordination of services to enhance community health. The role is integral to the integrated care team, supporting chronic care management and behavioral health needs while fostering connections with community resources and organizations.

Responsibilities

  • Establish trusting relationships with patients and their families while providing general support and encouragement.
  • Provide initial outreach and engage patients in their care plan goals specific to Social Determinants of Health (SDOH).
  • Conduct ongoing follow-up, basic motivational interviewing, and goal setting around identified SDOH needs with patients and their families.
  • Participate in an integrated team to support chronic care management and behavioral health needs.
  • Coordinate services and develop care plans as an active member of the patient's care team.
  • Conduct outreach efforts for newly assigned patients and those losing insurance coverage to help them stay covered and establish care at Orchid.
  • Be knowledgeable about community resources appropriate to the needs of patients/families and provide referrals.
  • Build and maintain relationships with community organizations and partners.
  • Document all patient contact in the EHR and record work performed no later than 24 hours after patient contact.
  • Exhibit excellent working relations with patients, visitors, staff, and community partners.

Requirements

  • Minimum of high school diploma or GED; higher education preferred.
  • One (1) year relevant experience working with underserved populations in a social service and/or community-based setting.
  • Basic computer skills, with preference for experience with EHR systems.
  • Ability to initiate and maintain positive working relationships with patients, staff, and other organizations.
  • Good communication skills, including active listening and appropriate language use.

Nice-to-haves

  • Certification as Traditional Healthcare Worker (THW) or Community Health Worker (CHW) preferred, but training will be compensated upon employment.
