About the position

The RN Case Manager at Lifespark plays a pivotal role in planning, organizing, and directing the services provided to clients, ensuring that all operations comply with regulatory requirements and adhere to Lifespark's best practices. This includes overseeing the starts of care, developing care plans, and providing ongoing support to clients. The RN Case Manager is also responsible for ensuring client satisfaction and supervising caregivers, which involves engaging with them and facilitating their onboarding process. In this role, you will conduct physical assessments and perform a variety of nursing duties, including medication management and treatments within the client's home environment. Additionally, you will provide education to clients, advocate for their needs, and counsel them and their families on available resources while initiating appropriate services. As a crucial member of the care team, the RN Case Manager is responsible for coordinating and delivering care to all clients on their caseload, ensuring that each individual receives the highest quality of care tailored to their specific needs.

Responsibilities

  • Plan, organize, and direct Lifespark services while ensuring compliance with regulatory requirements.
  • Oversee starts of care and develop care plans for clients.
  • Provide ongoing support and ensure client satisfaction.
  • Supervise and engage caregivers, facilitating their onboarding process.
  • Conduct physical assessments and perform nursing duties including medication management and treatments in clients' homes.
  • Provide education to clients and act as their advocate.
  • Counsel clients and families on available resources and initiate appropriate services.
  • Coordinate and deliver care for all clients on your caseload.

Requirements

  • Registered Nurse (RN) with a preference for a Bachelor of Science in Nursing (BSN).
  • Advanced certification as a Care Manager, Holistic Nursing, Geriatric Case Management, and/or Gerontology is preferred.
  • Minimum of 2 years of RN experience in community-based services such as assisted living, home care, hospice, or public health.
  • Experience working with seniors is preferred.
  • Experience in customer service or direct member contact is preferred.
  • Experience supervising caregivers is required.
  • Valid driver's license and auto insurance.

Nice-to-haves

  • Advanced certification in Care Management, Holistic Nursing, Geriatric Case Management, and/or Gerontology.
