About the position

The Community Housing Case Manager is responsible for program development and case management for transitional and permanent housing participants in specific counties. This full-time temporary position involves conducting interviews, developing case plans, and maintaining comprehensive records for participants, while also coordinating with community agencies to provide comprehensive services.

Responsibilities

  • Conduct interviews with potential participants.
  • Conduct life skill and budgeting sessions one-on-one or in groups.
  • Participate in the development and implementation of the Case Management program.
  • Provide ideas for programming specific to educational or process groups for participants.
  • Maintain a file on each participant that includes assessment, case plan, and documentation of progress.
  • Maintain internal systems of outcome tracking.
  • Maintain comprehensive statistical data on all participants and submit monthly summary of statistics.
  • Submit expenditure report forms and vouchers monthly.
  • Perform intakes that include required documentation for admission and client orientation.
  • Conduct written assessments on all new adult participants within 72 hours of admission.
  • Develop case plan with participant within 2 weeks of intake, including short- and long-term goals.
  • Provide information, referral, and financial assistance as needed.
  • Coordinate case management efforts with all staff and outside services.
  • Develop discharge plan with participants addressing supportive housing and economic stability.
  • Attend agency, community, supervisory, and clinical staffing meetings as needed.
  • Maintain working relationships with community agencies to provide comprehensive services.
  • Provide advocacy services and attend in-service trainings/workshops as needed.

Requirements

  • Bachelor's degree in social work or a related human service field required (i.e. psychology).
  • At least 2 years of case management experience preferred.
  • Great communication (verbal and written) skills.
  • Problem solving and conflict resolution skills.
  • Knowledge of computers and software programs.
  • Ability to work with diverse populations.
  • Great time management skills and ability to handle multiple demands and priorities.
  • Ability to interact positively with grantee, referral sources, and community partners.
  • Ability to comprehend, document, and converse in Standard English.
