About the position

The Community Liaison at Eden Health is responsible for supporting business development efforts across the Oregon Service area, focusing on census growth, brand recognition, and reputation for Home Health and Hospice services. This role involves maintaining relationships with referral sources, processing referrals, and ensuring effective communication between agency staff and referral sources to facilitate safe transitions for patients.

Responsibilities

  • Maintain relationships with referral sources including hospitals, long-term care facilities, rehabilitation centers, and physician practices.
  • Work closely with agency sales representatives to ensure effective communication and transition of referrals.
  • Process referrals according to agency and referring provider policies.
  • Explain Eden Health's program to patients, family members, and other referral sources accurately and thoroughly.
  • Collaborate with agency staff to ensure safe transitions for all patients to home.
  • Document all marketing activities accurately and timely as requested by the agency.
  • Follow up on referrals for evaluation and/or admission to Eden Health program.
  • Attend and participate in in-services, staff meetings, and case conferences.
  • Complete all documentation in an accurate, timely, and legible manner.
  • Obtain outstanding orders or documentation as requested by the agency.
  • Communicate effectively with patients, coworkers, and other healthcare professionals.
  • Participate in the development and implementation of processes in accordance with agency's evolving goals.
  • Meet and/or exceed home health and hospice referral and admission goals.

Requirements

  • Bachelor's degree in Marketing, Sales, Business, or Health Care preferred.
  • High school diploma or equivalent required.
  • Healthcare marketing experience in home health and/or hospice industry, DME, medical supplies outside sales, or pharmaceutical sales.
  • General clerical, data entry, and excellent organizational skills.
  • Ability to communicate effectively in English verbally and in writing.
  • Working knowledge of computer software programs.
  • Valid driver's license and reliable transportation.
  • Ability to pass a criminal background check and national sex offender clearance.
  • Must be able to physically navigate through the community for the entire shift.

Nice-to-haves

  • Experience calling on contacts at SNF, Hospitals, Dr. Offices, ALFs, and more.
