About the position

Horizon Health is seeking a Community Liaison Manager for our BRAND NEW unit called AlTru in Grand Forks, ND. This unit will be a 24 bed inpatient unit for both adult and pediatric patients, with plans to expand to an additional 24 beds in 2026. The Community Liaison Manager (CLM) will play a crucial role in the planning and implementation of a comprehensive community education program aimed at promoting the unit's behavioral health services. The CLM will coordinate the referral development and sales process, ensuring that accurate community education information and account management systems are maintained. Additionally, the CLM will oversee the unit's internal and external communications program, working closely with various stakeholders to enhance the visibility and effectiveness of the unit's services. The CLM will work in partnership with community-based providers, Horizon leadership, client hospital administrative staff, clinical staff, and unit leadership to develop and implement programs that promote the unit's behavioral health programs within designated areas and communities. This role requires a thorough understanding of the services provided by the program, including admission criteria and patient characteristics, to effectively communicate this information to current and potential referral sources, including medical staff and hospital clinical department managers. The CLM will also respond to requests for general information about the unit's treatment program and will be responsible for informing the Program Director, Vice President of Community Education, and Director of Operations of any adverse publicity. In addition to these responsibilities, the CLM will administer the management information system, ensuring accurate and timely reports are submitted as required. The CLM will evaluate data collected and utilize key reports to plan referral development and community education activities, as well as to assess the success of these efforts. The role also involves maintaining a comprehensive, categorized mailing list, monitoring all admissions, inquiries, and discharges, and entering data into the management system. The CLM will collaborate with health centers, community-based organizations, insurers, and community coalitions to develop programs, events, and new initiatives that align with marketing and unit program objectives. Finally, the CLM will coordinate referral source surveys in compliance with company standards and work with the Program Director to develop marketing plans for the program.

Responsibilities

  • Works in partnership with community-based providers, Horizon leadership, client hospital administrative staff, clinical staff and Unit leadership to develop and implement programs that promote Unit behavioral health programs within designated area/communities.
  • Administers the management information system and ensures accurate and timely reports are submitted as required.
  • Evaluates data collected and utilizes key HorizonPlus reports to plan referral development and community education activities and to determine the success of these efforts.
  • Maintains a thorough understanding of the services provided by the program, admission criteria and patient characteristics and effectively communicates this information to current and potential referral sources.
  • Responds to all requests for general information about the unit's treatment program and provides information about appropriate topics.
  • Informs Program Director, Vice President of Community Education and Director of Operations of any adverse publicity.
  • Coordinates and implements a focused account management system.
  • Develops, maintains, and continually updates a comprehensive, categorized mailing list.
  • Maintains a historical record of unit activities including news clips, photographs, and public relations in portfolio.
  • Monitors all admissions, inquiries and discharges and enters data into HorizonPlus.
  • Works with health centers, community-based organizations, insurers and community coalitions to develop programs, events and new initiatives that promote marketing and Unit program objectives.
  • Coordinates the referral source surveys in compliance with company standards.
  • Works with the Program Director to develop marketing plans for the program.
  • All other duties as assigned.

Requirements

  • Bachelor's degree required
  • 1-3 years of related experience
  • Proficient in Excel, Word, and PowerPoint
  • Excellent interpersonal, telephone, written and customer relations skills
