About the position

The Community Care Liaison at Elara Caring plays a crucial role in the growth of the home care industry by developing a dedicated team of healthcare providers and ensuring high-quality care for patients in their preferred home settings. This position involves strategic planning for territory growth, building relationships with referral sources, and collaborating with leadership to promote Elara Caring's services in the community.

Responsibilities

  • Creates and implements a strategic plan for territory growth and development.
  • Implements provider relations strategies to positively affect referral base and revenue in assigned territory.
  • Identifies and visits case managers, discharge planners, social workers, physicians, and other potential referral sources on a scheduled basis.
  • Documents efforts appropriately and submits planned schedules and reports to RVP and/or AVP.
  • Maintains knowledge of industry competitors to ensure market share retention and growth.
  • Stays informed about healthcare industry changes that may impact business operations.
  • Collaborates with leadership to plan and participate in trade shows and community events for referral generation.
  • Assists in contract assessment, preparation, and negotiation as necessary.

Requirements

  • Bachelor's Degree in Business Administration, Sales, or related field.
  • 5+ years' experience in healthcare sales.
  • Ability to effectively communicate in writing and verbally.
  • Ability to travel approximately 50% as business needs require.
  • Must have a dependable vehicle, valid driver's license, and current auto insurance.
  • Previous experience in home health or hospice is preferred.
