About the position

The Community Living Nurse is responsible for assessing and addressing the medical needs of individuals supported by Woodhaven. This role involves collaboration with community physicians and their staff to ensure effective care and compliance with medical recommendations. The nurse will also provide direct nursing care, manage health records, and support the overall health and safety of individuals with intellectual and developmental disabilities.

Responsibilities

  • Complete face-to-face monthly nursing assessments in compliance with DMH guidelines.
  • Review, analyze, and interpret lab results at least monthly.
  • Review Physician Orders for accuracy and staff compliance at least monthly.
  • Conduct monthly medication reconciliation and monitor side effects of medications.
  • Perform monthly health records review, assessing vital signs and other health indicators.
  • Delegate and supervise unlicensed support staff in medication administration and nursing tasks.
  • Complete RN Directives for program enhancements during monthly assessments.
  • Document monthly health assessments and billing notes in a timely manner.
  • Manage assigned case load and support other nursing colleagues.
  • Provide direct nursing care when needed, such as catheter changes and suture removal.
  • Advise management on health and safety issues of individuals.
  • Assess and counsel individuals who are ill or injured.
  • Share on-call responsibilities within the health services department.
  • Conduct employee in-services and training as needed.
  • Teach healthcare and illness prevention to individuals and their families.
  • Administer medications and treatments as prescribed.
  • Intervene appropriately with individuals displaying challenging behaviors.
  • Recommend supports and services as needed.
  • Maintain confidentiality of individual health information.
  • Serve as a member of the HIPAA team and protect private health information.
  • Collaborate with other agencies for the best interest of individuals supported.
  • Monitor the application of all physician orders and ensure compliance with DMH requirements.
  • Consult with various staff and agencies to emphasize best practices.
  • Coordinate and assist in the delivery of care plans with the health team.
  • Ensure compliance with CARF standards and certification principles.
  • Assess employees with work-related injuries and refer for medical follow-up.

Requirements

  • Valid Missouri Registered Nurse license.
  • Basic computer skills.
  • Experience in Community Health Care or working with individuals with intellectual/developmental disabilities.
  • Ability to maintain positive relationships with individuals supported.
  • Reliable personal vehicle for transportation.
  • Willingness to participate in on-call rotation.
  • Flexibility in work schedule and attendance.

Nice-to-haves

  • Experience in nursing with a focus on community health.
  • Knowledge of DMH guidelines and CARF standards.
