About the position

The Community Health Worker role focuses on outreach and education related to preventive health services, maternity care, and chronic condition management. This position involves significant field travel to engage with members and community organizations, fostering relationships that empower individuals to take charge of their health. The role requires collaboration with various service providers and the development of educational materials to enhance community health initiatives.

Responsibilities

  • Initiate outreach efforts related to targeted preventive health, maternity, care transitions, and chronic condition initiatives.
  • Educate members on the importance of targeted preventive health services and assess opportunities for care management intervention.
  • Make appropriate referrals, including to the Behavioral Health Managed Care Organization (BHMCO) as needed.
  • Contact members, providers, and community agencies to coordinate access to preventive health services.
  • Develop rapport with members and community centers to establish supportive relationships that empower members in their health and wellness.
  • Assist members with completing Health Risk Assessments and managing healthcare barriers such as knowledge deficits and transportation issues.
  • Collaborate with human services providers and state agencies to identify and outreach to members in need of services.
  • Provide accurate and current information about the Organization's lines of business and benefits to members in the community.
  • Contribute to the development and preparation of educational materials for members and providers.
  • Conduct provider and community training on EPSDT services and other preventive health initiatives, providing summaries and outcomes to management and peers.
  • Participate in the development of effective, measurable, and innovative programs targeting at-risk members.
  • Represent the company at community events.

Requirements

  • High School Diploma/GED
  • Act 33/34 and 73 clearances required (must obtain prior to starting assignment)
  • Knowledge of customer service principles and processes
  • Medical assistant experience
  • Call center experience
  • Ability to interact well with peers, supervisors, and customers
  • Problem-solving skills and ability to work independently
  • Good written and oral communication skills
  • Cultural sensitivity and appreciation for diversity, especially towards the Medicaid population
  • Understanding of healthcare-related issues faced by members, including transportation and financial barriers
  • Demonstrated patience and empathy in interactions with members.

Nice-to-haves

  • Experience in a healthcare-related customer service or marketing environment
  • Bilingual, Spanish speaking
  • Basic medical terminology background
  • Experience with the targeted community or population
