About the position

The Community Nurse is responsible for coordinating health care services for residents on a 24/7 basis, ensuring their health care needs are met through individual service planning and implementation. This role involves overseeing the work of House Managers and caregivers, providing clinical services, and ensuring compliance with health care regulations. The Community Nurse acts as an advocate for residents, maintains communication with families, and ensures high-quality care and customer satisfaction.

Responsibilities

  • Coordinate health care services for assigned residents on a 24/7 basis.
  • Participate in individual service planning and maintenance of these plans.
  • Recommend health care service goals and assist in their implementation.
  • Oversee and coordinate the work of House Managers and caregivers to meet resident needs.
  • Provide clinical services as needed and train staff to meet resident needs.
  • Coordinate delegation of medication administration and health care tasks per state regulations.
  • Notify physician and/or designated agents of resident status changes as appropriate.
  • Supervise and assist in the provision of all resident services, including those provided by contracted health care providers.
  • Conduct evaluations and develop service schedules based on resident needs.
  • Address the physical, social, spiritual, and psychological needs of residents.
  • Ensure timely assessments of residents and compliance with regulations related to care.
  • Administer medications and treatments based on individual service plans and document results.
  • Investigate complaints and ensure follow-up communication with complainants.
  • Maintain compliance with HIPAA, state regulations, and OSHA standards.
  • Conduct training for staff as assigned and mandated by regulatory requirements.

Requirements

  • Valid RN License required.
  • Ability to lift up to 50 pounds and 30 pounds regularly.
  • Ability to read, write, and communicate effectively.
  • Experience in coordinating health care services and working with residents.

Nice-to-haves

  • Experience in assisted living or long-term care settings.
  • Knowledge of state regulations and Nurse Practice Act.
