About the position

The Community Outreach Manager at ComForCare is a pivotal role designed for an ambitious and outgoing individual who is eager to build a career in community engagement and relationship management. This position is integral to the growth of our business, focusing on solidifying relationships within the community and creating additional pipelines of referrals that drive new business. The Community Outreach Manager will be responsible for developing and maintaining relationships with key referral sources, including case managers, social workers, physicians, and discharge planners from hospitals and nursing homes. This role requires regular visits and calls to these representatives to encourage referrals and keep them updated on clients sent to the agency. In addition to relationship management, the Community Outreach Manager will need to have a comprehensive understanding of the various programs offered by the agency, such as Dementia Wise and Matter of Balance, to effectively differentiate the agency in the marketplace. The role may also involve some direct consumer sales, with the ultimate goal of continuously increasing business. The ideal candidate will possess strong communication skills, both written and verbal, and will be able to build relationships across different levels of the organization. Creativity and the ability to develop innovative strategies for community engagement are essential, as is the self-discipline to drive results independently. The position requires the ability to give presentations to both large groups and individuals, making it crucial for the Community Outreach Manager to be comfortable in public speaking and engaging with diverse audiences. This role is not just about meeting targets; it’s about making a meaningful impact in the community and helping others live their best lives while ensuring that the agency's services are well-represented and understood.

Responsibilities

  • Meet with case managers, social workers, physicians, and hospital discharge planners to generate referrals.
  • Promote the agency within the community to increase visibility and referrals.
  • Develop and maintain relationships with major referral sources such as hospitals and senior communities.
  • Conduct research on new and untapped referral sources to expand outreach efforts.
  • Educate potential clients and families on home care services through sofa visits.
  • Deliver presentations to large groups and one-on-one to various audiences.
  • Promote care enhancement programs like Dementia Wise and Matter of Balance.
  • Meet key performance indicators (KPIs) set by the agency.
  • Perform other duties as assigned.

Requirements
