The Community Outreach Manager at ComForCare is a pivotal role designed for an ambitious and outgoing individual who is eager to build a career in community engagement and relationship management. This position is integral to the growth of our business, focusing on solidifying relationships within the community and creating additional pipelines of referrals that drive new business. The Community Outreach Manager will be responsible for developing and maintaining relationships with key referral sources, including case managers, social workers, physicians, and discharge planners from hospitals and nursing homes. This role requires regular visits and calls to these representatives to encourage referrals and keep them updated on clients sent to the agency. In addition to relationship management, the Community Outreach Manager will need to have a comprehensive understanding of the various programs offered by the agency, such as Dementia Wise and Matter of Balance, to effectively differentiate the agency in the marketplace. The role may also involve some direct consumer sales, with the ultimate goal of continuously increasing business. The ideal candidate will possess strong communication skills, both written and verbal, and will be able to build relationships across different levels of the organization. Creativity and the ability to develop innovative strategies for community engagement are essential, as is the self-discipline to drive results independently.
