The Community Outreach Manager is responsible for managing a regional team of Community Outreach Specialists while building local community relationships and engaging Medicare seniors to create organic patient growth in the local communities. The Community Outreach Representative will work very closely with local community partners and clinics to ensure seniors have access to value-based preventive primary care. The Community Outreach Manager will report to the Vice President of Marketing and Communications. In this role, you will deliver and coordinate educational presentations to existing and prospective patients, creating and maintaining working relationships with referral sources. You will attend lead-generating events in the community and at our clinics, engaging potential members through phone, text, and email. Managing and reporting on leads through CRM or other systems will be a key responsibility, along with health plan partner collaboration and coordination. You will develop and maintain local broker relationships and coordinate with marketing to promote events. Conducting prospective patient tours in partnership with operations and developing relationships with other community partners, such as non-profits, to acquire new patients will be essential. Collaboration with the operations and clinic staff to coordinate services/events for prospective patients, as well as working with the broker and local health plan community to increase awareness and patient enrollment, will be part of your daily activities. Additionally, you will collaborate with marketing to engage in social media and content development, working in a hybrid model that includes 50% field and home office work. Your ultimate goal will be to convert prospective patients to enrolled/active patients with booked visits and assigned primary care providers (PCP).
