About the position

Valley Community Services is dedicated to providing the highest quality services to individuals with intellectual disabilities. The Community Participation Supports Specialist plays a crucial role in this mission by ensuring that individuals receive the care and support they need in community settings. This part-time position, requiring 20 hours per week, involves working directly with assigned individuals to help them achieve their personal goals as outlined in their Individual Support Plans (ISPs). The specialist is responsible for fostering an environment that promotes dignity, privacy, and respect, while actively involving individuals in decisions about their care and support services. The role includes assisting individuals in acquiring, maintaining, and improving essential self-help, domestic, socialization, and adaptive skills necessary for successful living in home and community-based settings. This support encompasses various areas, including self-care, communication, mobility, and therapeutic activities. The specialist must ensure the health, safety, and welfare of the individuals by providing appropriate supervision and promptly reporting any concerns or significant changes in health or behavior. In addition to direct support responsibilities, the Community Participation Supports Specialist is expected to attend required staff meetings and training programs, complete necessary documentation such as time cards and progress notes, and coordinate effectively with team members and individuals. Adherence to Valley Community Services' policies and procedures is essential, and the specialist must perform their duties under the guidance of supervisors and professional staff. Responsibilities

  • Assists individuals in acquiring, maintaining, and improving self-help, domestic, socialization, and adaptive skills necessary to reside successfully in home and community-based settings.
  • Supports individuals in areas of self-care, communication, fine and gross motor skills, mobility, and therapeutic activities as detailed in the Individual Support Plan (ISP).
  • Ensures the health, safety, and welfare of individuals by providing supervision and reporting concerns and absences.
  • Attends required staff meetings and training programs.
  • Completes and submits all required documentation including time cards, progress notes, incident reports, and individual records.
  • Reports significant changes in health, safety, or behaviors both verbally and in writing.
  • Coordinates with the individual and team members on an ongoing basis in a professional manner.
  • Adheres to Valley Community Services Policies and Procedures under the guidance of supervisors and professional staff.

Requirements

  • Valid PA driver's license
  • High school diploma or GED equivalent
  • Background checks
  • Physical/Mantoux TB test required
