About the position

The Community Partners Supervisor is responsible for the development and management of the Community Partners team, focusing on individualizing community-based activities for participants based on their beliefs, interests, preferences, and support needs. This role aims to maximize participant inclusion in the community by leveraging community networking and support resources, while establishing professional relationships with both internal and external stakeholders to ensure effective communication and support.

Responsibilities

  • Collaborate with agency leadership to provide coaching and performance improvement plans for team members.
  • Participate in the hiring and onboarding process, ensuring team members understand their roles and expectations.
  • Assign and manage daily activities and routes for team members, ensuring adequate support is maintained.
  • Facilitate department functions including meetings, trainings, and incident reports.
  • Engage in site visits and attend monthly leadership meetings.
  • Review GPS alerts and schedule team members for corrective actions as needed.
  • Maintain accurate records for team members, including attendance and performance evaluations.
  • Review and approve weekly timesheets and mileage forms.
  • Participate in individual Pre-PCP meetings and ensure staff training on new PCPs.
  • Cultivate relationships with community members and organizations to achieve service goals.
  • Monitor crisis situations and adjust supports as needed while adhering to policies.
  • Develop and maintain information management systems to evaluate service outcomes.
  • Communicate urgent issues to the Assistant Director promptly.
  • Review goal documentation to support billing and monitor attendance tracking accuracy.
  • Collaborate with the Finance Department to ensure compliance with budgetary goals.

Requirements

  • High School Diploma required; Associate's or Bachelor's in Human Services preferred.
  • Two years of supervisory experience.
  • Three years of experience in the human services field.
  • Fluency in Microsoft Office Suite (Word, PowerPoint, Excel, Outlook).
  • Driver's License with a clean driving record.

Nice-to-haves

  • DSP I certification or ability to complete within the first 90 days.
  • Experience with assistive/adaptive technologies.
