About the position

The Community Partnership School Wellness Assistant Director is responsible for managing and integrating wellness programs and services within the Community Partnership Schools. This role serves as a case manager, addressing human and health needs through proactive behavioral health and case management solutions for students, staff, families, and the community. The position oversees wellness operations, manages referrals, and coordinates health and wellness events, ensuring compliance with policies and maintaining confidentiality.

Responsibilities

  • Manage and track the school health care programs referral network and processes.
  • Serve as the point of contact between UW CPS administration, school nurse, medical clinic, behavioral health department and other health related agencies/providers.
  • Manage and direct all medical and clinic referrals, including health, dental, vision, and mental health referrals.
  • Participate in Intervention Team meetings to provide information on behavioral interventions and relevant referrals as needed.
  • Track and follow up on all referrals made in and out of school.
  • Establish referral network/linkages with outside partner agencies.
  • Track and record professional assessments, treatment plans, and progress notes.
  • Participate in the quality improvement and progress monitoring process.
  • Follow and enforce the school district's and wellness partner's policies and procedures.
  • Maintain and update the relevant databases throughout Outlook, iCarol, spreadsheets, and Andar.
  • Manage and implement wellness activities to engage families to participate in activities and services on campus and in the community.
  • Participate in trainings and site visits related to the Community Partnership Schools development.
  • Utilize data management system for proper documentation.
  • Manage and coordinate Health & Wellness events for students, families, staff & communities.
  • Develop a strong knowledge base and stay current on health and wellness issues and trends.
  • Maintain confidentiality and adhere to HIPAA regulations.
  • Manage annual calendar of events to ensure all activities, programs and challenges are implemented in a consistent and timely manner.
  • Oversee health and wellness operations across all United Way Community Partnership Schools.
  • Oversee the Wellness Committee and Clinic Advisory meetings.
  • Responsible for onboarding, managing, and training the Wellness Coordinator.

Requirements

  • Bachelor's degree in Social Work, Psychology, Counseling, or Nursing.
  • One (1) year post-Bachelor's degree experience working with children and families.
  • Excellent active listening and interpersonal skills.
  • Strong problem solving, critical thinking, prioritization and judgment skills.
  • Business writing skills are necessary.
  • Strong administrative and operational skills essential.
  • Ability to write reports and business correspondence.
  • Ability to present information to diverse groups.

Nice-to-haves

  • Experience working in a school environment.
  • Knowledge of Volunteer and Relationship Management, Community Relations and Project Management.
