About the position

Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as a health plan, healthcare provider, and social center designed for older adults with complex medical needs. As a Community RN at Florida PACE Centers, you will work under the general supervision of the clinic manager RN and medical director. Your primary responsibility will be to provide nursing care to all persons enrolled in the program. This includes performing comprehensive nursing histories and physical assessments on new referrals, creating appropriate care plans, and participating in interdisciplinary team (IDT) meetings. You will evaluate and triage participants with episodic illnesses and refer them to the medical director or medical specialists as indicated. Active participation in family conferences and interdisciplinary team meetings, committees, and work groups is also expected. The role requires a strong commitment to ensuring that residents' and patients' rights are adhered to, demonstrating professionalism and accountability, and maintaining a caring attitude consistent with Miami Jewish EmpathicareSM. You will need to demonstrate excellent communication skills, ensure confidentiality and security of patients' medical information, and be able to think and act calmly in unusual occurrences. Responsibilities

Requirements

  • Graduate from an accredited school of nursing.
  • At least 1 year of RN or 3 years of LPN required.
  • Minimum one-year experience working with a frail/elderly population.
  • Current RN license by the Florida State Board of Nursing.
  • Valid Florida Drivers License & Proof of Insurance.
  • BLS certification required.
  • ACLS preferred.
  • Bilingual English/Spanish, knowledge of Creole a plus.
  • Proven clinical leadership.
  • Ability to get along with others in an interdisciplinary team environment.
