About the position

The Community School General Case Manager (GCM) plays a crucial role in delivering long-term prevention case management services to youth and their families who face barriers to regular school attendance, academic achievement, and overall well-being. The GCM is responsible for managing a caseload of approximately 35 youth, collaborating with key school personnel and community supports to assist referred youth through case planning, in-school interventions, and ongoing in-home case management services. This position emphasizes purposeful collaboration with the Community School Coordinator, school attendance team, counselors, and other support staff to effectively promote positive change for the youth being served. The GCM's responsibilities include assessing the needs and strengths of referred youth to develop, revise, and monitor individualized plans aimed at achieving the goals of the youth and their families. The GCM is dedicated to removing or lessening barriers to success, which may involve accompanying youth and their parents or guardians to appointments, as well as assisting with the completion and submission of necessary documentation. The role requires an empathic, compassionate, and creative approach, as the GCM will work as part of a team of case managers who are cross-trained to perform both GCM and Attendance Case Manager duties, ensuring a seamless transition for families navigating available supports at the community school. In addition to direct case management, the GCM is expected to engage with other school youth periodically to foster collaborative efforts that support the overall mission of the program. The position requires a commitment to adhering to all relevant policies and procedures, timely submission of required documentation, and active participation in meetings and training sessions to enhance service delivery and program effectiveness.

Responsibilities

  • Deliver long-term prevention case management services to approximately 35 youth and their families.
  • Collaborate with school personnel and community supports to assist referred youth with case planning and interventions.
  • Engage with other school youth to support collaborative efforts.
  • Assess the needs and strengths of referred youth to inform individualized plan development.
  • Remove or lessen barriers to success for referred youth and families.
  • Accompany youth/parents/guardians to appointments as needed.
  • Monitor assigned youth's school attendance and academic success through regular contact with school personnel.
  • Complete necessary documentation for accessing emergency funds for assigned youth.
  • Facilitate and monitor referrals made for identified youth and/or family members.
  • Participate in team meetings and biweekly supervisory sessions.

Requirements

  • Bachelor's Degree in Social Work or a related field.
  • One year of direct service experience with parents and children in a community setting.
  • Valid driver's license and proof of car insurance, with daily access to a vehicle for travel.
  • Ability to work independently and be highly motivated.
  • Excellent written and oral communication skills.
  • Strong time management skills and creative problem-solving abilities.
  • Proficiency in Microsoft Office Suite.
  • Ability to relate and communicate effectively with adults and youth.
  • Knowledge of demographics and culture of the community school and surrounding areas.
  • Expert understanding of School District of Philadelphia attendance policies and procedures.
