About the position

The Community Support Assistant position at Continuum of Care involves providing comprehensive community-based residential and support services to individuals with mental health, intellectual, and addiction needs. The role focuses on enhancing clients' independence and well-being through hands-on skill-building interventions and collaboration with Community Support Specialists. The position is available in various shifts and offers both part-time and full-time opportunities.

Responsibilities

  • Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Support clients' integration into the community by providing resources for securing and maintaining affordable housing.
  • Maintain regular contact with clients as defined and assist with referrals for various services.
  • Develop, implement, and monitor person-centered plans with clients in all aspects of care.
  • Enhance clients' independent living skills through various teaching modalities.
  • Assist clients in identifying barriers to achieving their goals and link them to appropriate services.
  • Conduct regular apartment/room inspections and assist clients in maintaining cleanliness.
  • Develop and maintain working relationships with families and external providers.
  • Facilitate psycho-educational groups as needed and administer breathalyzer tests and urinalysis samples as applicable.
  • Attend and participate in staff meetings and provide additional case management support as needed.

Requirements

  • Bachelor's Degree preferred; High School Diploma or equivalent with a minimum of 3 years' experience in a related field.
  • Prior experience working with people with severe and persistent mental illness, intellectual disabilities, and/or addiction.
  • Valid Connecticut Driver's license with a clean driving record is required.
  • First Aid/CPR certification or willingness to obtain certification within 90 days of hire.
  • Proficient computer skills required, including Microsoft Office and electronic record keeping.

Nice-to-haves

  • Experience in direct care support for at least 1 year (preferred).
  • Experience in mental health or intellectual disabilities for at least 1 year (preferred).
  • Experience in addiction support for at least 1 year (preferred).
  • First Aid Certification (preferred).
  • CPR Certification (preferred).
