About the position

The Community Support Guide position is a part-time role focused on empowering individuals with intellectual disabilities to achieve their personal goals. This involves providing transportation to clients, assisting them in community engagement activities, and helping them develop skills necessary for their independence. The role emphasizes compassion, patience, and kindness while working closely with clients to facilitate their aspirations.

Responsibilities

• Transport clients to and from community locations to work on their specific goals.
• Spend at least two hours with each client to help them achieve their aims.
• Assist clients in accomplishing goals stated in their service plan developed by the Client's Case Manager.
• Develop strategies with the family to connect the Client with community resources based on their interests and needs.
• Facilitate connections between the Client and local community members for mutual benefits and activities.
• Gather information to enhance Client participation in their community and provide written resources for future reference.
• Identify service goals and summarize the Client's progress towards achieving these goals in measurable terms.
• Provide progress notes after each session and quarterly.

Requirements

• High School Diploma or GED required.
• Bachelor's, Master's or Doctoral degree in Psychology, Sociology, Social Work, Education, or as a Certified Life Skills Coach preferred.
• At least one year of experience working with individuals with intellectual disabilities is required.
• Must have a vehicle and up-to-date vehicle insurance.

Nice-to-haves

• Experience in community engagement or social work is a plus.
