About the position

The Community Support Peer position focuses on providing community-based support to individuals with behavioral health challenges. The role involves assisting clients in developing life skills, managing daily activities, and ensuring continuity of care during crises. The position requires active engagement with clients, outreach to at-risk individuals, and collaboration with various service providers to facilitate access to necessary resources and support services.

Responsibilities

  • Provide community-based support to build on independent activities of daily living such as grocery shopping, attending medical appointments, and assisting with medication reminders.
  • Facilitate communication externally and across agency staff to ensure continuity of care for individuals presenting in crisis services.
  • Provide comprehensive consumer care and case management services, including access to transportation services, medical care, and housing support.
  • Teach life-management and daily living skills to clients that translate to their natural environment with an emphasis on developing natural supports.
  • Conduct outreach to individuals at risk of entering crisis systems of care to assess access barriers and engage them into treatment.
  • Maintain networking and referral contacts with other human service providers.
  • Complete all required forms and documentation in a timely manner.
  • Demonstrate active involvement in team meetings, supervision, and community events.
  • Record and document consumer treatment following established policies and procedures.
  • Operate as the initial access point for individuals calling the agency's 24/7 resource and crisis line.
  • Assess presenting needs and the immediate safety of individuals in crisis; facilitate warm handoff to Crisis QMHPs and/or first responders.
  • Provide phone-based peer support to individuals not in need of a face-to-face response.
  • Field inquiries from the Illinois Call4Calm line to assess immediate needs and link to resources.
  • Provide off-site peer support and stabilization services during crisis intervention when necessary.

Requirements

  • Experience in behavioral health or related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of community resources and support services.
  • Experience in crisis intervention and case management.

Nice-to-haves

  • Certification in peer support or related training.
  • Experience working with individuals with mental health or substance use disorders.
  • Bilingual skills.
