About the position

The Community Support Professionals/Case Managers play a vital role in providing support to children, youth, and adults who are facing serious mental health challenges. This contract position requires professionals to engage directly with clients in various settings, including their homes, schools, and community locations. The primary focus is on skill building and facilitating access to essential services such as medical, psychiatric, and educational support. The role demands a proactive approach to meet clients where they are, ensuring that they receive the necessary resources to aid in their recovery journey. In this position, Community Support Professionals/Case Managers are responsible for assessing the needs of their clients and identifying appropriate resources to address those needs. This involves conducting thorough needs assessments and collaborating with clients to create personalized goals. The professionals will advocate for their clients, teaching and demonstrating new skills that promote functional independence. They will also coordinate with various internal and external entities, including therapists, doctors, schools, and legal representatives, to ensure a comprehensive support system is in place. Additionally, the role includes responsibilities such as discharge and transition planning, attending treatment meetings, and accompanying clients to community-based appointments. Daily documentation of client interactions and progress is required, along with participation in weekly treatment team meetings to discuss client status and strategies for ongoing support. This position is essential for fostering a supportive environment that empowers clients to achieve their recovery goals and improve their overall quality of life.

Responsibilities

  • Provide support for clients with serious mental health conditions and connect them with the services and resources needed to meet their recovery goals.
  • Assess client needs and identify adequate resources to meet those needs.
  • Conduct needs assessments and create goals with clients, providing assistance through advocacy, teaching, and role modeling new skills.
  • Coordinate with internal and external entities such as therapists, doctors, schools, and the legal system.
  • Participate in discharge and transition planning for clients.
  • Attend specific treatment meetings and staffings to discuss client progress and strategies.
  • Accompany clients to community-based appointments as needed.
  • Complete required daily documentation of client interactions and progress.
  • Attend weekly treatment team meetings to discuss client status and support strategies.

Requirements

  • Completion of a bachelor's degree from an accredited college or university in social work, psychology, counseling, or criminal justice is preferred.
  • Direct experience in a related field can substitute for educational requirements.
  • Ability to meet clients in various community settings, including homes, schools, and offices.
  • Strong communication and interpersonal skills to effectively engage with clients and coordinate with service providers.
