About the position

The Community Support Provider - Adult (Case Manager) position is a full-time role based in Lisbon, OH, offering a hybrid work environment. This position is integral to providing direct services to clients and their families, focusing on needs assessment, treatment planning, skill development, monitoring, linking, referral, networking, advocacy, and crisis intervention. The role requires the individual to engage in service planning, which includes case coordination and the documentation of assessments and progress towards treatment goals. The successful candidate will be expected to meet productivity expectations on a fiscal basis, which involves attending team and staff meetings, receiving supervision, and participating in local travel and training as necessary. In this role, the Community Support Provider will be responsible for timely and accurate completion of electronic documentation and will need to work a flexible schedule based on agency and client needs, as determined in collaboration with their supervisor. The position also requires computer data entry, and proficiency in Microsoft Office, particularly Word, is preferred. The candidate must be prepared to undergo criminal background checks, BMV checks, and drug screening, and must possess a valid driver's license with a good driving record. Responsibilities

  • Provide direct service to clients and families, including needs assessment and treatment planning.
  • Develop skills in clients and monitor their progress towards treatment goals.

Requirements

  • Bachelor's degree in mental health, social work, counseling, psychology, or human/family services.
  • Two years of experience in a related field if the degree is in a different area such as education or criminal justice.
  • Preferred: Bachelor's degree in a related field of mental health with one year of paid experience, preferably with the service population.
  • Valid LSW license in Ohio is preferred.
  • Skills in written and verbal communication, assessment and planning, organization, interpersonal skills, and teamwork.
  • Proficiency in computer data entry and knowledge of Microsoft Office, especially Word, is preferred.
  • Familiarity with electronic health records (EHR) is preferred.

Nice-to-haves

  • Experience with the service population is preferred.
