The Community Support Resource Specialist at Optima Medical Management Group plays a crucial role in enhancing the quality of life for individuals by providing comprehensive support and care. This position focuses on collaborating with community members to ensure they receive necessary assistance, tracking and reporting activities related to resources, and facilitating connections between members and support networks. The role emphasizes building relationships with providers and community organizations to address barriers and promote well-being.

Responsibilities

• Collaborate closely with Community Support in the ECM Program, coordinating with CS Outreach Specialists and ensuring comprehensive resource coverage.
• Initiate connections with new resource opportunities via phone and email, and conduct meetings to educate staff about community support services.
• Cultivate and maintain relationships with providers and stay informed about public assistance programs.
• Communicate with CS Outreach Specialists or case managers to ensure members are connected to proper resources based on their specific needs.
• Collaborate with Business Development to procure resources and establish efficient intake processes for new resources.
• Conduct periodic visits to community sites to foster relationships and promote ongoing communication with community service providers.

Requirements

• Valid California driver's license and valid vehicle insurance.
• High school diploma or GED.
• Minimum of 1 year experience in healthcare, care coordination, medical terminology, health education, or customer service.
• Excellent communication, interpersonal, customer service, and organizational skills.
• Computer skills for documentation, email, and chat support.
• Proficient in multitasking, organization, and attention to detail.

Nice-to-haves

• Experience in public speaking or presentations.
• Familiarity with community support services.
