About the position

The Community Support Services Case Manager at Preferred Behavioral Health Group plays a crucial role in assisting consumers in achieving and maintaining valued life roles in various aspects such as employment, education, housing, and social environments. This position is part of a dynamic and innovative non-profit organization dedicated to providing trauma-informed care and fostering a diverse and inclusive workplace.

Responsibilities

• Contributes to the development, implementation, monitoring, and updating of the Individualized Rehabilitation Plan.
• Conducts therapeutic rehabilitative skill development training to improve consumers' functional, social, interpersonal, problem-solving, coping, and communication skills.
• Performs skill development training targeting daily living skills, social skills, accessing healthcare services, public entitlements, recreational activities, self-advocacy, and wellness.
• Performs crisis intervention activities as necessary and observes consumers' symptoms to assist with management.
• Communicates observations and information about the consumer to the Licensed Master's level CSS or RN.
• Assists consumers with locating, furnishing, and moving to housing.
• Provides training and support with consumers' finances, such as budgeting and opening bank accounts.
• Educates consumers on available transportation resources.

Requirements

• High School Diploma or Bachelor's degree in a helping profession (social work, human services, counseling, psychiatric rehabilitation, psychology).
• Minimum of 5 years of professional experience in a community behavioral healthcare setting for those with a High School Diploma.
• Certification as a psychiatric rehabilitation practitioner may substitute for one year of experience.
• Valid Driver's License in good standing.
• Basic computer skills.
