About the position

Community Alliance, offering an integrated health care approach and built on an array of mental health and primary care services, is seeking a Community Support Specialist I. Ideal candidates will have a passion for working with vulnerable populations and be comfortable collaborating with other professionals to empower clients to achieve self-defined goals. The Community Support Specialist I is responsible for administering and delivering community-based mental health services, case management, and identifying community resources for adults with serious mental illness. This role involves developing and coordinating necessary services and support within the community, assessing individual mental health, co-occurring, and functional strengths and needs. The specialist will utilize motivational interviewing strategies and techniques throughout the assessment, individual planning, and rehabilitative process. Additionally, the Community Support Specialist I will assist clients in obtaining and maintaining suitable housing, plan and provide training related to personal, community living, and social skills, and assist in transition planning, discharge processes, and the delivery of follow-up services for individuals leaving the program.

Responsibilities

  • Administer and deliver community-based mental health services.
  • Provide case management for adults with serious mental illness.
  • Identify community resources to support clients.
  • Develop and coordinate necessary services and support within the community.
  • Assess individual mental health, co-occurring, and functional strengths and needs.
  • Utilize motivational interviewing strategies throughout the assessment and planning process.
  • Assist clients in obtaining and maintaining suitable housing.

  • Assist in transition planning and discharge processes for individuals leaving the program.
  • Deliver follow-up services for clients post-discharge.

Requirements

  • Bachelor's Degree in Human Services or related field, or two years post High School coursework in a Human Services field plus two years relevant experience/training.
  • Demonstrated knowledge and skills in mental health and substance abuse rehabilitation and recovery principles.
  • Experience in service delivery and behavior intervention/management practices at a direct service level.
  • Basic knowledge of characteristics and needs of persons with serious mental illness, including co-occurring disorders.
  • Ability to maintain effective working relationships with diverse individuals and groups.
  • Possession of a valid driver's license and a good driving record.
