About the position

The Community Support Specialist for the Adult Outpatient Substance Use Program at Places for People, Inc. is a full-time role focused on empowering individuals and their families to achieve long-term wellbeing and independence. This position involves working directly with clients experiencing substance use issues and supporting them through all phases of recovery by connecting them with community resources and coordinating care. The ideal candidate will possess strong communication skills and a passion for helping others.

Responsibilities

  • Provide case management services to adults with co-occurring substance use and mental health disorders, including assessment, treatment planning, and intervention implementation.
  • Collaborate with clients, families, and community stakeholders to support the implementation of individualized treatment plans that address the client's specific needs and goals.
  • Utilize a strengths-based approach that emphasizes empowerment, skill-building, and resilience, incorporating evidence-based practices such as Motivational Interviewing, Trauma-Informed Strategies, and skills based in CBT and DBT.
  • Coordinate care with other service providers, including primary care physicians, psychiatrists, schools, and community organizations.
  • Maintain accurate and timely records of client interactions, treatments, and progress, in accordance with organizational policies and procedures.
  • Participate in staff development activities, including training and education programs, to enhance knowledge and skills related to substance use and mental health disorders.
  • Work collaboratively with other members of the team, including therapists and support staff to ensure effective coordination of care and support for clients.

Requirements

  • Bachelor's degree in Psychology, Social Work, or other human services specialized field of study; OR an unrelated Bachelor's degree with two years of related work experience; OR any four-year combination of higher education and two years of related work experience; OR an Associate's of Applied Science in Behavioral Health Support; OR four years of qualifying experience in delivering services to individuals with mental health disorders, substance use disorders, or developmental disabilities.
  • Experience providing one-on-one or group services with a recovery/resiliency focus.
  • Ability to teach and model coping strategies for individuals managing psychiatric and/or substance use disorder issues.
  • Experience assisting individuals in achieving goals and objectives in their individual treatment plans.
  • Must be at least twenty-one (21) years of age to operate a company-owned vehicle and twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van.
  • Current driver's license, acceptable driving record, and current auto insurance.

Nice-to-haves

  • Knowledge of policies, procedures, and regulations of the program.
  • Experience with Motivational Interviewing and Trauma-Informed Strategies.
