About the position

Hopewell Center currently has an opportunity available for a Community Support Specialist (CSS) in our Adult Services Department. This position is responsible for providing activities and interactions that enable an individual at clear risk of hospitalization, or who was discharged from inpatient treatment to maintain an adjustment to living in the community. The CSS is also responsible for evaluation of client's status including personal strengths, areas of concern and needs. The CSS worker will participate in formal assessment and the development of the person-centered plan; monitor client's progress towards goal attainment; provide assistance and act as a 24-hour resource when necessary. Train clients in living skills, social skills and other pro-social activities which enable clients to actively participate in the recovery process. The CSS worker also transports clients to appointments and makes house visits as applicable. The CSS worker is an active member of the Clinical Treatment Team. CSS worker is responsible for maintaining client records and assuring standards and guidelines are upheld.

Responsibilities

  • Provide activities and interactions to support individuals at risk of hospitalization or recently discharged from inpatient treatment.
  • Evaluate client's status, including personal strengths, areas of concern, and needs.
  • Participate in formal assessments and the development of person-centered plans.
  • Monitor client's progress towards goal attainment and provide assistance as needed.
  • Act as a 24-hour resource for clients when necessary.
  • Train clients in living skills, social skills, and other pro-social activities to aid in their recovery process.
  • Transport clients to appointments and conduct house visits as applicable.
  • Maintain client records and ensure compliance with standards and guidelines.

Requirements

  • Bachelor's degree in a human services field (social work, psychology, nursing, education, criminal justice, recreational therapy, human development and family studies, counseling, child development, gerontology, sociology, human services, behavioral science, rehabilitation counseling).
  • Any four-year degree with two years of qualifying experience.
  • A combination of four years of higher education and qualifying experience.
  • Four years of qualifying experience in service delivery to individuals with mental illness, substance abuse, or developmental disabilities.
  • Experience providing one-on-one or group services, teaching and modeling coping and management skills.
  • Experience supporting individuals in finding and maintaining employment and functioning appropriately in families, schools, and communities.
  • Experience assisting individuals in achieving goals on their individualized treatment or person-centered plans.
  • Valid Class E driver's license, dependable transportation, clean driver's record, and personal vehicle insurance.
