About the position

The Community Support Specialist - Assertive Community Treatment (ACT) at Carle Health is responsible for providing comprehensive behavioral health assessment, treatment planning, and case management for individuals with serious mental illnesses. This role involves mobile crisis response, individual and group support, and coordination with various community resources to ensure clients receive the necessary care and support to manage their mental health effectively. The specialist will work under the supervision of the ACT Supervisor and is expected to represent the agency professionally while functioning as an integral team member.

Responsibilities

  • Provides community-based/in-home behavioral health services for individuals experiencing serious mental illness.
  • Completes assessment and treatment plan with clients.
  • Provides on-call interventions for ACT clients, covering after business hours, overnights, and weekends as assigned.
  • Conducts crisis assessment and intervention, completing safety plans with clients and following up after crisis situations.
  • Coordinates with Emergency Response Service and other crisis service teams when clinically appropriate.
  • Provides individual and group ACT services, including monitoring daily living activities and assisting with benefits and entitlements.
  • Maintains records in accordance with TrilliumPlace policies and state and federal regulations.
  • Acts as a liaison between the community and client needs, coordinating treatment goals with various agencies.
  • Participates in in-service and competency training and interdisciplinary team meetings.

Requirements

  • Bachelor's Degree in counseling, social work, psychology, or a related field.
  • Two years of documented clinical experience in a mental health setting under the supervision of a QMHP.
  • High school diploma or GED with a Certificate in psychiatric rehabilitation from a DHS-approved program.
  • Driver's License upon hire.
  • Ability to delegate, exercise good judgment, and maintain interpersonal relationships.

Nice-to-haves

  • Experience in crisis intervention and mobile crisis response.
