About the position

The Community Support Specialist for the Assertive Community Treatment (ACT) team plays a crucial role in providing rehabilitation and support services to individuals with chronic mental health disorders. This position involves collaborating with clients and treatment teams to develop and implement treatment plans, providing crisis intervention, and ensuring clients receive comprehensive care within their communities. The ACT team operates 24/7, delivering essential services that promote wellness and recovery in a supportive environment.

Responsibilities

  • Collaborate with the client and treatment team in developing, writing, implementing, evaluating and revising overall treatment goals and plans.
  • Participate in and conduct parts of the client-centered comprehensive assessment of psychiatric history, mental status and diagnosis, physical and dental health, and social development.
  • Provide service coordination (case management) for an assigned group of clients and advocate for clients' rights and preferences.
  • Provide individualized supports in areas of symptom management, dual diagnosis substance abuse services, and activities of daily living.
  • Provide practical help and supports, advocacy, coordination, and training to help clients obtain necessities of daily living.
  • Attend daily staff organizational meetings and treatment planning review meetings, and weekly supervision.
  • Complete all required documentation according to agency policy, including Comprehensive Assessments and Progress Notes.
  • Provide on-call crisis intervention covering nighttime, evening and weekend hours.
  • Consult with community agencies and families to maintain coordination in the treatment process.
  • Assist clients with transportation as clinically indicated.

Requirements

  • High School/GED required.
  • Associates, Bachelor's or Master's degree in a human service related field preferred.
  • At least four (4) years of experience required if no degree has been obtained.
  • At least two (2) years of experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves

  • Experience working in community mental health settings.
  • Knowledge of substance use disorders and treatment options.
  • Skills in crisis intervention and conflict resolution.
