About the position

The Community Support Specialist is responsible for providing community-based healthcare services for individuals dealing with substance abuse, mental health issues, and co-occurring disorders. The role focuses on maximizing client independence and promoting recovery through personalized care plans, active participation, and support systems. The specialist will work closely with clients to develop and implement strategies that enhance their adjustment and functioning within the community, ensuring they receive the necessary resources and support for sustained recovery.

Responsibilities

  • Provide holistic, person-centered care emphasizing individual strengths and skills acquisition.
  • Ensure all treatment plans are continually updated based on client records.
  • Facilitate recovery/resiliency through activities that define and attain recovery goals.
  • Develop and implement individualized care plans with measurable interventions.
  • Initiate and maintain contacts with referral and third-party payer sources as required by policy.
  • Document services clearly, detailing the need for services and the interventions provided.
  • Develop discharge and aftercare plans to secure successful transitions to continued services.
  • Provide education to consumers and families about managing mental illness and substance use disorders.
  • Help individuals develop skills to address symptoms that interfere with job maintenance.
  • Model professionalism to achieve high service standards.

Requirements

  • Bachelor's Degree in social work, psychology, nursing, or a closely related field, or 3 years of qualifying experience in a related field.
  • Current certification in CPR and First Aid, or ability to achieve certification within 60 days of employment.
  • Valid driver's license and automobile insurance, with insurability under the agency's motor vehicle insurance policy.
  • Experience in providing one-on-one or group services focused on rehabilitation and recovery.

Nice-to-haves

  • Experience in teaching and modeling coping strategies for psychiatric or substance use issues.
  • Knowledge of current resources available for individuals with substance use disorders.
  • Ability to work effectively with diverse cultures and backgrounds.
