About the position

The Community Support Specialist is responsible for providing community-based healthcare services for individuals dealing with substance abuse, mental health issues, and co-occurring disorders. The role focuses on maximizing client independence and responsibility while promoting recovery and resilience through personalized care plans and support systems. The specialist will work closely with clients to facilitate their adjustment and functioning within the community, ensuring they receive the necessary services to achieve sobriety and sustained recovery.

Responsibilities

  • Provide holistic, person-centered care emphasizing strengths and skills acquisition.
  • Ensure all treatment plans are continually updated based on client records.
  • Facilitate recovery/resiliency through activities that define and attain recovery goals.
  • Develop, implement, and revise care plans with specific, measurable interventions.
  • Initiate and maintain contacts with referral and third-party payer sources as required.
  • Document services clearly, detailing the need for services and interventions provided.
  • Develop discharge and aftercare plans to ensure successful transitions to continued services.
  • Provide education to consumers and families about managing mental illness and substance use disorders.
  • Help individuals develop skills to address symptoms that interfere with job maintenance.
  • Model professionalism to achieve high service standards.

Requirements

  • Bachelor's Degree in social work, psychology, nursing, or a closely related field, or 3 years of qualifying experience.
  • Current certification in CPR and First Aid, or ability to achieve certification within 60 days of employment.
  • Experience providing one-on-one or group services with a rehabilitation focus.
  • Ability to negotiate complex service systems for participants.
  • Excellent written and verbal communication skills.
  • Ability to work autonomously or with minimal direction.

Nice-to-haves

  • Experience in addiction counseling and harm reduction strategies.
  • Supervising experience in a healthcare or community support setting.
