Join our team and make a positive impact on the lives of others! We are looking for a Community Support Specialist for our growing team in Massachusetts. This role is field-based and will require you to travel at least 3-4 times a week. There will also be potential travel to our FreedomCare hub closest to you. The Community Support Specialist will perform in-home visits in the home of the Adult Foster Care member to assist them in starting on services with FreedomCare. You will guide them through the necessary paperwork, provide guidance on using our FreedomCare mobile app, and start their services. Once the member is On Care, the Community Support Specialist will assist the Care Manager and LPN in conducting in-home visits to ensure they are receiving quality service, and address any concerns the caregiver or patient has. Adult Foster Care is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient's independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient.

Responsibilities

  • Perform in-home visits to assist Adult Foster Care members in starting services with FreedomCare.
  • Guide members through necessary paperwork and provide guidance on using the FreedomCare mobile app.
  • Assist Care Manager and LPN in conducting in-home visits to ensure quality service.
  • Participate in the development, implementation, and ongoing review of the AFC plan of care with the member.
  • Complete progress notes corresponding with each on-site visit and encounter with members.
  • Provide appropriate health education, information, support, and counseling to caregivers/patients as required per case.
  • Engage in interdisciplinary meetings with care managers, caregivers, and patients.
  • Collaborate in the care practice and communicate impressions of cases to appropriate staff.
  • Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers.

Requirements

  • Two or more years of experience working with elders or disabled adults and caregivers.
  • Previous field experience in a medical or case management environment, preferred.
  • Experience with data input, case review/case note entry; Salesforce experience is a plus.
  • Working knowledge of ADL/IADL assessments.
  • Valid driver's license with an insured vehicle for travel within the state.
  • Excellent verbal and written communication, organizational and time management skills.
  • Strong critical thinking and problem-solving skills.
  • Proficient in Microsoft (i.e., Word, Excel, Outlook).
