About the position

The Community Support Specialist (CSS) plays a vital role in assisting individuals and families in need of housing and rental assistance. This position focuses on providing case management and supportive services to ensure stability in clients' homes, including home and community-based visits. The CSS will work collaboratively with clients to develop service plans and utilize community resources effectively.

Responsibilities

• Provides case management and supportive services for clients, including social skills, life skills, and job search assistance.
• Participates in setting case plan goals and objectives with clients.
• Completes case plans and logs, ensuring all documentation meets regulatory standards.
• Conducts home and community-based visits to assess client needs and provide support.
• Maintains a clear driving record to transport clients as needed.
• Effectively utilizes community resources by providing appropriate referrals and linkages.

Requirements

• Must be over 18 years of age.
• Possess a valid driver's license and a vehicle for client visits.
• Entry-level position with a focus on case management and supportive services.
