About the position

The Community Support Specialist (CSS) plays a vital role in assisting individuals and families in need of housing and rental assistance. This position involves providing case management and supportive services to ensure stability in clients' homes, including home and community-based visits. The CSS will work closely with clients to develop service plans and connect them with community resources, promoting recovery and independence.

Responsibilities

• Assist individuals and families in locating housing and providing supportive services.
• Provide case management and supportive services including social skills, life skills, and job search assistance.
• Participate in setting case plan goals and objectives with clients.
• Complete case plans and maintain accurate documentation of client services.
• Ensure clinical documentation meets applicable standards for licensing and accreditation.
• Perform regular case maintenance activities, including documenting actions and preparing reports.
• Utilize community resources effectively by providing appropriate referrals and linkages.
• Maintain a clear driving record to transport clients as needed.

Requirements

• Must be over 18 years of age.
• Possess a valid driver's license and a vehicle for client visits.

Nice-to-haves
